To attend PREP practice sessions:
  • PAR-Q - Printed, handwritten form, bring with you to PREP practice session
  • PARmed-X - This form required if you are over 45 and not accustomed to regular physical activity, further questions need to be answered through the Par-Med X form, to participate in the prep session. You must bring with you the signed Par-Med X form by your physician with no restrictions to participate in the physical portion of the session,

Immunizations

The following immunizations are required prior to the date of hire:

  • Tetanus, Diphtheria and Polio (TdP) or Tetanus and Diphtheria (Td) or Diphtheria, Tetanus and acellular Pertussis (dTaP)
  • Measles, Mumps and Rubella (MMR)
  • Hepatitis B series
  • Two-Step Tuberculin Skin Test (Mantoux)
  • Chest X-Ray (only if Mantoux > = 10mm)

Driving Records

To obtain a record of current demerit point accumulations, you must attend your local Service Ontario kiosk or centre and request a driver abstract.

To obtain an Extended Driving Record (over 5 years), a request must be made by letter to:
