- PAR-Q - Printed, handwritten form, bring with you to PREP practice session
- PARmed-X - This form required if you are over 45 and not accustomed to regular physical activity, further questions need to be answered through the Par-Med X form, to participate in the prep session. You must bring with you the signed Par-Med X form by your physician with no restrictions to participate in the physical portion of the session,

Immunizations
The following immunizations are required prior to the date of hire:
- Tetanus, Diphtheria and Polio (TdP) or Tetanus and Diphtheria (Td) or Diphtheria, Tetanus and acellular Pertussis (dTaP)
- Measles, Mumps and Rubella (MMR)
- Hepatitis B series
- Two-Step Tuberculin Skin Test (Mantoux)
- Chest X-Ray (only if Mantoux > = 10mm)
