Accessing Police Reports

A number of police reports can be provided to individuals or organizations requiring information to substantiate claims to insurance companies or other agencies.

• Collision Reports, for motor vehicle collisions reported to Toronto Police Service
   
• Occurrence Summary/Confirmation letters provided in order to verify that a report has been made with the Toronto Police Service and provide general information pertaining to that report
   
• Lost Passport reports, for foreign passports only, Canadian government does not require this report

How to Apply
 

In Person

Please visit our Customer Service Centre, located at Toronto Police Headquarters – 40 College Street, Monday through Friday between 7 a.m. and 4:30 p.m. Note, requests are not accepted at Police Divisions.

• Request forms are available at the Customer Service Counter or on our website

• Please ensure that you bring the following with you:

  1. Two (2) pieces of government identification (at least one with a photograph)

  2. Applicable fee (refer to Fee Schedule or below)

     • Occurrence Report Summary/Confirmation Letter– $67.80*

     • Motor Vehicle Collision Report – $67.80

     • Collision Field Notes/Witness Statements– $135.60

     • Lost/Stolen Passport Report Summary/Confirmation Letter– $39.55

  3. All fees are payable with Visa, Mastercard, Amex or Interac. Please note, we do not accept cash at our Customer Service Centre. You may also pay with a certified cheque or money order made payable to the Toronto Police Service.

  4. Please provide as much information as possible pertaining to the report.

     • Officer’s name and badge number

     • Report number

     • Date of occurrence/collision

     • Location – road, street name, or intersecting streets

     • Names of involved parties

     • Plate numbers of vehicles involved

     • Type of occurrence

By Mail

Requests can be submitted by mail.

• Either send in a completed request form or write a letter indicating the nature of your request

• Please ensure that you include the following with your letter or request form:

  1. A photocopy of two pieces of government identification (at least one with a photograph)

  2. Applicable fee (refer to Fee Schedule or below)

     1. Lost/Stolen Passport Report Summary/Confirmation Letter- $39.55

     2. Collision Field Notes/Witness Statements -$135.60

     3. Collision Report - $67.80

     4. Occurrence Report Summary/Confirmation Letter - $67.80*

  3. All fees are payable with a certified cheque or money order made payable to the Toronto Police Service

  4. Please provide as much information as possible pertaining to the report

     • Type of occurrence

     • Plate numbers of vehicles involved

     • Names of involved parties

     • Location – road, street number or intersecting street

     • Date of occurrence/collision

     • Report number

     • Officer’s name and badge number

* The occurrence request processing fee is waived for out-of-town visitors (those not residing in the Greater Toronto Area), Students (actively enrolled in a High School or Post-Secondary Institution), and Seniors. Please note, the processing fee cannot be waived for requests for a Collision Report or Collision Field Notes/Witness Statements.
