If you have been involved in a motor vehicle collision you may require a copy of the collision report for insurance purposes or simply for your records. The Toronto Police Service only provides Motor Vehicle Collision (MVC) reports for collisions that occur in Toronto. Any reports relating to collisions that occurred outside of Toronto should be requested through the respective police service in that jurisdiction.

Please note that a fee of $67.80 (taxes included) applies.

Requests may be submitted by mail or in person. Please refer to the How to Apply section below for information on how to submit a request.

Collisions that occurred on any 400 series highways within Toronto would be investigated by the OPP. Reports for collisions that occurred on the 427, 401, 400, 404, and Queen Elizabeth Expressway can be obtained through the Ontario Provincial Police.

Please note, collision reports are retained for six years after which they are destroyed and no longer available (with the exception of collisions resulting in a fatality, which are retained indefinitely).

How to Apply
 

In Person

Please visit our Customer Service Centre, located at Toronto Police Headquarters – 40 College Street, Monday through Friday between 7 a.m. and 4:30 p.m. Note, requests are not accepted at Police Divisions.

  • Request forms are available at the Customer Service Counter or below

  • Please ensure that you bring the following with you:

    1. Two (2) pieces of government identification (at least one with a photograph)

    2. Applicable fee for Motor Vehicle Collision Report – $67.80

    3. All fees are payable with Visa, Mastercard, Amex or Interac. Please note, we do not accept cash at our Customer Service Centre. You may also pay with a certified cheque or money order made payable to the Toronto Police Service.

    4. Please provide as much information as possible pertaining to the report.

      • Officer’s name and badge number

      • Report number

      • Date of collision

      • Location – road, street name, or intersecting streets

      • Names of involved parties

      • Plate numbers of vehicles involved

 

By Mail

Requests can be submitted by mail.

  • Either send in a completed request form or write a letter indicating the nature of your request

  • Please ensure that you include the following with your letter or request form:

    1. A photocopy of two pieces of government identification (at least one with a photograph)

    2. Applicable fee for Collision Report - $67.80

    3. All fees are payable with a certified cheque or money order made payable to the Toronto Police Service

    4. Please provide as much information as possible pertaining to the report

      • Plate numbers of vehicles involved

      • Names of involved parties

      • Location – road, street number or intersecting street

      • Date of occurrence/collision

      • Report number

      • Officer’s name and badge number
