Confirmation letters are provided in order to verify that a report has been made with the Toronto Police Service and provide general information pertaining to that report.  If you require documentation to substantiate the fact that an occurrence has been reported to the Police, this document may be useful.

If you are making a request on behalf of an insurance company or law firm you must provide a written authorization from your client in order to obtain a confirmation letter.

Requests may be submitted by mail or in person.  Please refer to the How to Apply section below for information on how to submit a request.

 


 

If you are the victim of any of the following you are eligible to apply for a Confirmation Letter:

  • Domestic
  • Assault related offences
  • Threatening/Harassment
  • Robbery

There is no fee for the above confirmation letters.  The letter is provided within 10 business days and can be either mailed or picked up at Police Headquarters when ready.

The letter will provide general information relating to the occurrence including:

  • The date of the occurrence
  • Type of Occurrence
  • Location of Occurrence
  • Your information (Person that made the report) - Victim
  • Preparing officer’s names and badge numbers

 


 

If you are the victim of any of the following you are eligible to apply for an Occurrence Report Summary Confirmation Letter. Please note that a fee of $67.80 (taxes included) applies*. This letter is often useful for insurance purposes.

  • Theft
  • Theft of Auto
  • Theft from Auto
  • Break and Enter
  • Lost Property
  • Fraud
  • Fire/Arson
  • Mischief (Vandalism)

The letter will provide general information relating to the occurrence including:

  • The date of the occurrence
  • Type of Occurrence
  • Location of Occurrence
  • Your information - Victim
  • Preparing officer’s names and badge numbers
  • Injuries if applicable
  • Brief summary including information relating to any damage to property and items that may have been lost or stolen. This is not a complete report.**

* The request processing fee is waived for out-of-town visitors (Not residing in the Greater GTA Area), Students (actively enrolled in a High School or Post-Secondary Institution), and Seniors.

** Release of full Police Reports are done in accordance with Municipal Freedom of Information and Protection of Privacy Act (MFIPPA); please refer to the application process of the Access and Privacy Section of the Toronto Police Service.

How to Apply
 

In Person

Please visit our Customer Service Centre, located at Toronto Police Headquarters – 40 College Street, Monday through Friday between 7 a.m. and 4:30 p.m. Note, requests are not accepted at Police Divisions.

  • Request forms are available at the Customer Service Centre

  • Please ensure that you bring the following with you:

    1. Two (2) pieces of government identification (at least one with a photograph)

    2. Applicable fee for Occurence Report Summary/Confirmation Letter– $67.80*

      All fees are payable with Visa, Mastercard, Amex or Interac. Please note, we do not accept cash at our Customer Service Centre. You may also pay with a certified cheque or money order made payable to the Toronto Police Service.
    3. Please provide as much information as possible pertaining to the report.

      • Officer’s name and badge number

      • Report number

      • Date of occurrence

      • Location – road, street name, or intersecting streets

      • Names of involved parties

      • Type of occurrence

 

By Mail

Requests can be submitted by mail.

  • Either send in a completed request form or write a letter indicating the nature of your request

  • Please ensure that you include the following with your letter or request form:

    1. A photocopy of two pieces of government identification (at least one with a photograph)

    2. Applicable fee: Occurence Report Summary/Confirmation Letter - $67.80*

    3. Fee is payable with a certified cheque or money order made payable to the Toronto Police Service

    4. Please provide as much information as possible pertaining to the report

      • Type of occurrence

      • Plate numbers of vehicles involved (if applicable)

      • Names of involved parties

      • Location – road, street number or intersecting street

      • Date of occurrence

      • Report number

      • Officer’s name and badge number
