If your passport has been lost or stolen, it may be necessary to provide of a copy of this report to the Embassy or Consular Office of the Passport issuing country. Once you have made a report with the Toronto Police concerning this matter, you may submit a request for this service.

A fee of $39.55* is applicable for this service. However, the fee is waived for visitors from outside the Greater Toronto Area, Students and Seniors.

We do not issue a physical copy of a lost passport report, however the report is emailed or faxed to the respective consular office/embassy upon receipt of above fee and a written request.

If you have lost a Canadian Passport, please note that the Toronto Police Service does not create reports for lost Canadian Passports. Lost Canadian Passports/travel Documents must be reported to the Canadian Passport Program by calling 1-800-567-6868, or in person at the nearest Service Canada Passport Office.

Outside of Canada, a report should be made at the nearest Government of Canada Office.

In Person

Please visit our Customer Service Centre, located at Toronto Police Headquarters – 40 College Street, Monday through Friday between 7 a.m. and 4:30 p.m. Note, requests are not accepted at Police Divisions.

  • Request forms are available at the Customer Service Centre

  • Please ensure that you bring the following with you:

    1. Two (2) pieces of government identification (at least one with a photograph)

    2. Applicable fee for Lost/Stolen Passport Report Summary/Confirmation Letter– $39.55

    3. All fees are payable with Visa, Mastercard, Amex or Interac. Please note, we do not accept cash at our Customer Service Centre. You may also pay with a certified cheque or money order made payable to the Toronto Police Service.

    4. Please provide as much information as possible pertaining to the report.

      • Officer’s name and badge number

      • Report number

      • Date of occurrence

 

By Mail

Requests can be submitted by mail.

  • Either send in a completed request form or write a letter indicating the nature of your request

  • Please ensure that you include the following with your letter or request form:

    1. A photocopy of two pieces of government identification (at least one with a photograph)

    2. Applicable fee for Lost/Stolen Passport Report Summary/Confirmation Letter- $39.55. All fees are payable with a certified cheque or money order made payable to the Toronto Police Service

    3. Please provide as much information as possible pertaining to the report
