Create a Supplemental Report

You can use this report to submit additional information to your existing report.

When commencing the report, please choose the correct supplemental report type:

• The initial report was submitted online

OR

• The incident was reported to a police officer, either in person or over the phone, and they created a report

You can attach up to three (3) additional electronic items, limited to JPEGs, Documents and PDFs, that do not exceed 10MB each.
